Transportation Service Description
The Avon Senior Center Transportation Program (PDF) is an unassisted, curb-to-curb service available to Avon's older adult residents who have no alternative means of transportation.
A Passenger Transportation Information and Waiver form needs to be completed and returned before your first journey with Avon Senior
Transportation Services.
Requests for transportation to radiation, chemotherapy, physical therapy, dialysis, etc. will be taken as scheduling allows.
The transport van is wheelchair accessible.
Appointments are scheduled on a first-come, first-serve basis. Special priority is given to medical appointments.
